In the "Description" field of the web form, please be sure to include these details:
- Approximate purchase date
- Name of store where it was purchased
- Are you the person who purchased the water appliance originally? Do you still have your proof of purchase?
- Brand of the water appliance
- Product finish (chrome, brushed nickel, etc)
- Your full name
- Mailing address - including zip code
- Contact phone number - including area code
- The issue you are having with the water appliance, the more details the better
- The model number from the box, manual or receipt. Otherwise, a picture of the entire unit so that we can identify it.
Please click "Submit" to complete the submission. A Request received confirmation will be sent to you with the ticket number via the email address entered. This number allows us to quickly identify your communication with us.
If the matter is urgent, please call in with the ticket number and any one of our agents will be able to retrieve your picture(s) and supporting information to assist you.
